Program Name: Anchor Family Healthcare Patient Visit Summary Messaging
Program Description: Anchor Family Healthcare sends after-visit summary text messages to patients who have requested and consented to receive them. Messages may include visit follow-up instructions, medication information, lab or test information, and next appointment details. This is a transactional messaging program; it is not used for marketing or promotional purposes.
How Patients Enroll: Patients provide verbal consent during their in-person office visit to receive their after-visit summary by text message. Patients are advised at the time of consent that text messaging is not a fully secure communication channel. No automated or recurring subscription is created; each message is sent individually following a patient's visit, at the patient's request.
Message Frequency: Message frequency varies and depends on the number of visits a patient has. Patients generally receive one message following an office visit.
Cost: Message and data rates may apply, depending on the patient's mobile carrier and plan. Anchor Family Healthcare does not charge patients for these messages.
